Surgeon's Error
A Surgeon’s Error is what a botched operation leaves in a corpse. When removing or putting in an organ ends badly on the Autopsy Table, the slot gets a Surgeon’s Error instead of the organ. There is one for each of the six organs, and each counts as −1 red and −1 white skull (see Corpses and organs).
It never reaches your backpack, and the slot can no longer be opened, so it stays in the body for good. A body with one is missing an organ as far as the Resurrection Table I is concerned, so it can no longer become a zombie.

How to obtain
Failed autopsy
Left in the corpse in place of the organ when removing or implanting one fails — see Corpses.
Notes
- For a grave it cuts both ways. Losing the organ and gaining the error both take a red skull off, which helps, but the white skull it takes off lowers the grave’s ceiling.
- Every botched removal, insertion or zombie swap counts toward Oops Surgeon, whether or not it leaves an error behind.
- Swapping an organ in a zombie never leaves one. A failed swap just loses the new organ.
